本人(等)拟以下列题目:□原着,□病例报告,□专题报导,□继续教育型式刊登,申请投稿於「台湾医学」杂志.题目:「」

本篇过去未曾发表於其他杂志,且同意在贵杂志接受审查期间及接受刊登後,不投刊其他杂志,同时遵守贵杂志投稿规则,如违背愿受贵志编辑委员会裁决处理.

本篇列名之着者皆为实际参与研究及撰述,并能担负修改,校对与审查者讨论之工作,投稿前所有签名着者均仔细过目并同意论文之内容及结论.(列为着者之原则,请参阅台湾医学会杂志1988,vol.87,No.2,"如何写作者栏和致谢栏"一文)

Publictrustinthepeerreviewprocessandthecredibilityofpublishedarticlesdependinpartonhowwellconflictofinterestishandledduringwriting,peerreview,andeditorialdecisionmaking.Conflictofinterestexistswhenanauthor(ortheauthor'sinstitution),reviewer,oreditorhainancialorpersonalrelationshipsthatinappropriatelyinfluence(bias)hisorheractions(suchrelationshipsarealsoknownasdualmitments,petinginterests,orpetingloyalties).Theserelationshipsvaryfromthosewithnegligiblepotentialtothosewithgreatpotentialtoinfluencejudgment,andnotallrelationshipsrepresenttrueconflictofinterest.Thepotentialforconflictofinterestcanexistwhetherornotanindividualbelievesthattherelationshipaffectshisorherscientificjudgment.Financialrelationships(suchasemployment,consultancies,stockownership,honoraria,paidexperttestimony)arethemosteasilyidentifiableconflictsofinterestandthemostlikelytounderminethecredibilityofthejournal,theauthors,andofscienceitself.However,conflictscanoccurforotherreasons,suchaspersonalrelationships,academicpetition,andintellectualpassion.

InformedConsent

Patientshearighttoprivacythatshouldnotbeinfringedwithoutinformedconsent.Identifyinginformation,includingpatients'names,initials,orhospitalnumbers,shouldnotbepublishedinwrittendescriptions,photographs,andpedigreesunlesstheinformationisessentialforscientificpurposesandthepatient(orparentorguardian)giveswritteninformedconsentforpublication.Informedconsentforthispurposerequiresthatapatientwhoisidentifiablebeshownthemanuscripttobepublished.AuthorsshouldidentifyIndividualswhoprovidewritingassistanceanddisclosethefundingsourceforthisassistance.

Identifyingdetailsshouldbeomittediftheyarenotessential.Completeanonymityisdifficulttoachieve,however,andinformedconsentshouldbeobtainedifthereisanydoubt.Forexample,maskingtheeyeregioninphotographsofpatientsisinadequateprotectionofanonymity.Ifidentifyingcharacteristicsarealteredtoprotectanonymity,suchasingeicpedigrees,authorsshouldprovideassurancethatalterationsdonotdistortscientificmeaningandeditorsshouldsonote.

Therequirementforinformedconsentshouldbeincludedinthejournal'sinstructionorauthors.Wheninformedconsenthasbeenobtaineditshouldbeindicatedinthepublishedarticle.
